The Process
From your first inquiry to your first appointment, Eura makes it simple to get started.
Fill out the form above or call 646.535.3772. Include what type of care you’re looking for so we can prepare for your visit.
Eura will contact you within one business day to confirm a time and answer any questions about insurance, cost, or what to expect.
You’ll receive a link to your patient portal where you can complete your health history and intake questionnaire before your visit.
Takes about 10–15 minutes. Complete it at least 24 hours before your appointment.
Your appointment is conducted via secure video call. You’ll meet your provider, discuss your goals, and create your care plan together.
Lab orders and prescriptions are sent electronically. Most patients complete labs at a nearby Quest or LabCorp — no appointment needed.
Be Prepared
Having these items available before your first appointment helps your visit go smoothly.
Your member ID and group number. Eura’s billing team will verify your benefits before your visit.
A government-issued ID is required to create your patient record. A photo on your phone is fine.
All current medications, doses, and how long you’ve been taking them — including supplements.
Any relevant bloodwork from the past 12 months helps your provider plan your care and may reduce day-one lab costs.
Name, address, and phone number of the pharmacy where you’d like prescriptions sent.
A card is required to hold your appointment. Self-pay patients are charged at time of visit; insured patients pay their copay.
Policies
Eura holds appointment slots for you. Please give as much notice as possible if you need to cancel or reschedule.

Common Questions
In most cases, no. Eura accepts direct-access appointments. Some insurance plans require a referral — Eura will advise you during intake if that applies to your plan.
Eura is currently licensed to see patients in New York, New Jersey, Connecticut, and Colorado via telemedicine.
Initial consultations are 45–60 minutes. Follow-up visits are approximately 30 minutes.
A smartphone, tablet, or computer with a camera and microphone. Eura uses a HIPAA-compliant video platform — no app download required, just a link sent before your visit.
Yes. Eura is in-network with Anthem BCBS (CT & NY), UnitedHealthcare CT, Horizon BCBS NJ, and Aetna. Self-pay rates and superbills are available for other plans. See all accepted plans.
Lab orders are sent electronically to Quest Diagnostics, LabCorp, or another lab of your choice. Walk in with your order — no appointment needed at most locations.
All information is protected under HIPAA. Eura uses a secure, HIPAA-compliant patient portal (ChARM Health) for all records, messaging, and appointments. Eura never shares information without your explicit consent.
