Eura Health PLLC (“Eura Health,” “we,” “us,” or “our”) is a licensed medical practice providing predominantly telehealth services — affirming, evidence-based healthcare — to patients located in New York, New Jersey, Connecticut, and Colorado. Services are delivered by Theodore R. Miller, FNP-BC, AAHIVS, and any licensed providers working within Eura Health PLLC.
By accessing or using the Eura Health website (eurahealth.com), submitting any intake or contact form, scheduling an appointment, or receiving services from Eura Health, you agree to be bound by these Terms of Service and our Privacy Policy. If you do not agree, do not use this website or our services.
Eura Health provides telehealth medical services including but not limited to: HIV care, PrEP and HIV prevention, gender-affirming hormone therapy, STI testing and treatment, mental health support, and birth control management. All clinical care is delivered by licensed healthcare providers in accordance with applicable state law.
Eura Health is licensed to provide telehealth services only to patients who are physically located in New York, New Jersey, Connecticut, or Colorado at the time of their visit. By scheduling and attending an appointment, you confirm that you are physically present in one of these states.
Eura Health providers reserve the right to decline to prescribe medications, order tests, or provide referrals based on their professional clinical judgment. Receipt of services is not guaranteed. A provider may determine that telehealth is not appropriate for your clinical situation and refer you to in-person care.

Appointments may be scheduled through the Eura Health website, patient portal, or by contacting our office directly. Appointment availability is not guaranteed.
We ask that you provide at least 24 hours’ notice to cancel or reschedule an appointment. Late cancellations and no-shows may be subject to a fee as described in your patient intake paperwork. Repeated no-shows may result in discharge from the practice.
You are responsible for ensuring you have a functioning device with a camera, microphone, and reliable internet connection for telehealth visits. Eura Health is not responsible for technical failures on the patient’s end.
Eura Health accepts select insurance plans. It is your responsibility to verify that your insurance plan is accepted and that your benefits cover the services you receive. Coverage determinations are made by your insurer, not Eura Health.
You are responsible for any amounts not covered by insurance, including copays, deductibles, coinsurance, and fees for non-covered services. Eura Health will provide a good-faith estimate of costs upon request.
Consistent with the federal No Surprises Act, Eura Health will provide uninsured and self-pay patients with a good-faith cost estimate before scheduled services. If you have questions about your estimated costs, contact us atsupport@eurahealth.com.
You may use the Eura Health website for lawful purposes related to learning about and accessing our healthcare services. You may not use the site to transmit harmful, false, or misleading content, or in any way that violates applicable law.
Content on the Eura Health website is provided for general informational purposes only and does not constitute medical advice. It does not create a provider-patient relationship. Always consult a qualified healthcare provider for medical concerns.
All content on this website — including text, graphics, logos, and page design — is the property of Eura Health PLLC and is protected by applicable copyright and trademark law. You may not reproduce, distribute, or create derivative works without our written permission.
Your use of Eura Health’s services is also governed by ourNotice of Privacy Practices, which describes how we collect, use, and protect your health information in accordance with HIPAA. By using our services, you acknowledge that you have read and understood our privacy practices.
Information submitted through the Eura Health contact form is not encrypted and should not include sensitive medical information. Do not use the contact form to communicate urgent health concerns. Secure messaging is available through the patient portal for established patients.
The Eura Health website and its content are provided “as is” without warranties of any kind, express or implied. Eura Health does not warrant that the website will be uninterrupted, error-free, or free of harmful components. We make no warranty regarding the accuracy or completeness of any general health information on this site.
To the fullest extent permitted by law, Eura Health PLLC and its providers shall not be liable for any indirect, incidental, consequential, or punitive damages arising from your use of this website or our services. Our total liability for any claim shall not exceed the amount you paid to Eura Health in the three months preceding the claim. Nothing in these terms limits liability for gross negligence, willful misconduct, or claims that cannot be limited under applicable law.
Eura Health reserves the right to discontinue care to any patient with reasonable advance notice, except in circumstances where immediate termination is warranted (e.g., patient misconduct, fraudulent activity, or patient is no longer located in a licensed state). Patients who are discharged from the practice will be provided with information to facilitate transfer of care.
These Terms of Service are governed by the laws of the State of New York, without regard to conflict of law principles. Any disputes arising under these terms shall be resolved in the state or federal courts located in New York County, New York.
Eura Health may update these Terms of Service from time to time. Material changes will be communicated to active patients via email or patient portal notification. Your continued use of Eura Health’s services after changes are posted constitutes acceptance of the revised terms.
Questions about these Terms of Service may be directed to:
- Provide accurate and complete information about your health history, current medications, and symptoms.
- Disclose all relevant information that may affect your care, including medications prescribed by other providers.
- Be physically located in a licensed state (NY, NJ, CT, or CO) at the time of each appointment.
- Attend scheduled appointments on time or provide adequate notice of cancellation.
- Use a private, secure location for telehealth visits where you can speak freely.
- Follow the care plan and instructions provided by your Eura Health provider.
- Seek emergency services if you believe you are experiencing a medical emergency.
